Turbo 350 question

I am rebuilding a T350, 1977 model year.

These parts were missing upon disassembly: couple of the servo/accumulator springs, governor filter screen, only two 1/4" balls in the VB. So the trans has been apart before.

Low/rev, for & dir clutches, bushes, etc looked in good nick, all the frictions still had the waffle pattern, so little to no wear.

The int clutch had one severely warped steel,[ not referring to the waved plate ], other steels looked ok. Frictions still had the waffle pattern. Endplay was about 0.050".
All the rubber seals were still soft & in the correct positions, the five sealing rings on the stator shaft were intact, metal type with interlocking ends.

Any idea why this one plate warped?

Maybe a used plate that found its way in during assembly?

The missing parts sounds like it had a shift kit installed.
