I have done a guide in a guide in a guide. Big block chevy. .502 guide in original press in guides, then a liner in that guide. worked fine. Not sure if that would be the same sorta thing. I think the flanged sleeve would be the way to do it.

My thoughts are the possibility of poor heat transfer between the two sleeves and block.
I would have to think about a coating of heat transfer paste on both sleeves at installation, making sure to maximize thermal contact between the sleeves. This could be awkward if heating/cooling parts for assembly/press fit.
